Understanding Semaglutide, Ozempic, Wegovy, and Mounjaro

Semaglutide is the active ingredient in Ozempic and Wegovy, both brand-name medications. Ozempic is FDA-approved for type 2 diabetes but may be prescribed off-label for weight loss, while Wegovy is FDA–approved for weight loss. Semaglutide is also available as a generic medication. 

Mounjaro contains a different active ingredient: tirzepatide. Both tirzepatide and semaglutide are classified as GLP-1 receptor agonists. 

GLP-1 agonists work in a few different ways to support weight loss:

  • Blood sugar regulation and lipolysis

GLP-1 agonists trigger insulin secretion and slow down the release of glucagon. Insulin and glucagon are both hormones that regulate blood sugar levels. By affecting these hormones, GLP-1s lower blood sugar levels and stimulate lipolysis, the body’s natural fat-burning process. 

  • Hunger signaling

Is Semaglutide Safe and Effective?

The FDA studied and confirmed semaglutide’s safety and efficacy in four 68-week trials. For those determined to be candidates, semaglutide is considered safe with medical supervision, although side effects are possible. The most common side effects are headache, fatigue, dehydration, stomach pain, nausea, vomiting, burping, bloating, diarrhea, and indigestion.

Am I a Candidate for Semaglutide?

Semaglutide (Wegovy) is FDA-approved for individuals with a BMI of 30 or higher and those with a BMI of at least 27 with weight-related medical problems (i.e., high blood pressure or diabetes). It’s generally recommended for people who meet this requirement and haven’t achieved long-term weight loss with lifestyle changes alone. 

You may not be a candidate for liraglutide for weight loss if you have a history of:

  • Gallbladder, kidney, or pancreas conditions
  • Thyroid disease or endocrine tumors
  • Stomach or intestinal problems
  • Depression, mental health disease, or suicidal thoughts
  • Eye diseases
  • A known allergy to semaglutide or one of its components

What Diet Should I Follow with Medical Weight Loss Injections?

Your diet still matters with a medical weight loss injection. The injection alone is not enough to optimize your weight; it must be combined with a well-balanced diet.

Additionally, GLP-1 injections may lead to gastrointestinal side effects like nausea, gas, and bloating. A well-rounded diet won’t exacerbate (and may even prevent) these symptoms, making it easier to stick to your weight loss plan. 

The recommended diet for most individuals on GLP-1 agonist medications includes:

  • Low-starch vegetables, including zucchini, leafy greens, and mushrooms

How Fast Do You Start Losing Weight with Medical Weight Loss Injections?

The speed at which medical weight loss injections work will vary depending on the patient. For some people, semaglutide starts working within the first week.

However, most people start to experience shifts in their appetite and minor weight changes within the first few weeks of using GLP-1 agonist injections. Within four to five months of starting the injections, you’ll likely experience most of the weight loss. 

Semaglutide has various effects that are classified as short-term, medium-term, or long-term.

  • The short-term effects of semaglutide include reduced appetite and increased satiety after meals or snacks. Many people experience these effects within the first four weeks. 
  • The medium-term effects of semaglutide include notable weight loss and a more dramatic decrease in appetite. This generally occurs five to 20 weeks after starting the injections.
